RURAL HEALTH TRANSFORMATION

You Earned the CMS funding. Now your RHT program has to produce the numbers.

Rural Health Transformation funding pays for programs, not the outreach, workflows, and infrastructure they need to succeed.

Tapp Network helps funded organizations and state teams build and operate that layer, so data is ready when reporting begins.

  • Hands on RHT Strategy Workshop
  • Customized Go-To Market Roadmap
  • $5,000 value, fully funded by Tapp Network
  • Ten nonprofits selected per quarter.

WHO WE WORK WITH
Organizations Executing The Grant

You're delivering a funded program, but the award may not cover what it takes to run it.

We help health systems, coalitions, FQHCs, universities, workforce boards, community organizations, and other subrecipients turn funded initiatives into working programs.

How the workshop works for funded organizations
State Teams Managing The Grant

You coordinate a network of agencies, vendors, and subrecipients. Your results depend on what they deliver.

We work directly with state teams, alongside strategy consultants, or under a prime contractor to strengthen implementation across the portfolio.

THE REPORTING CLOCK

Half of every future year's funding rides on a score. The score rides on data.

CMS recalculates each state's technical score annually based on implementation progress and performance against committed metrics. That score determines half of the state's future allotment.

Every funded organization's data contributes to that result. Building the right reporting infrastructure early signals readiness and protects the program from having to reconstruct results later.

THE GAP

Platforms do not run programs. Data, People & Systems do.

Your state may have selected referral networks, telehealth tools, workforce systems, data platforms, and consultants. Between those investments sits the work that determines whether the program succeeds:

The workshop maps the operational layer before your organization commits budget or reaches its first reporting deadline.

FROM ROADMAP TO IMPLEMENTATION

Your grant funds the program. We help build the operations behind it.

Implementation may include:
Outreach and Enrollment

Campaigns, screening, intake, and retention workflows that help rural residents find and participate in programs.

Communications and Automation

CRM-driven enrollment sequences, reminders, participant follow-up, partner communications, and re-engagement.

Partner and Network Management

Partner onboarding, coalition communications, shared intake tools, and reporting support for smaller organizations.

Data and Reporting Infrastructure

Connections between program platforms, CRM data, participant activity, and state reporting requirements.

Workforce Program Operations

Recruitment, training coordination, credentialing, placement, employer engagement, and retention tracking.

Ongoing Operations

Monthly management of campaigns, workflows, integrations, and reporting cycles, allowing program staff to stay focused on participants.

FOR STATE TEAMS

Your technical score depends on what your funded network can produce.

Tapp supports state programs in three ways:
01
Support for Subrecipients

States can direct funded organizations to the workshop as an early step toward stronger enrollment, engagement, and data collection.

02
Delivery Under a Prime

We provide engagement systems, integrations, and reporting infrastructure inside an existing consulting or program-management engagement.

03
Direct Statewide Implementation

We bid directly on workforce platforms, multi-agency engagement environments, coalition systems, and other scopes aligned with infrastructure we already operate.

Every Rural Health Transformation plan will invest in care delivery, workforce, technology, and data. Lasting impact will depend on how well those investments are connected. States need an operating layer that coordinates the people doing the work, engages the residents the work is intended to serve, and measures whether the work is producing results.
